Looks like REPEAT LAST action doesn't work properly for editing mode. In particular for extrude operation. When you repeat last extion for extrude - it will allways moves towerd first one extrusion oriented movemant and never toward 'normals'. Open default file. Put a cube. Turn on edit mode. Select any side and extrude it. Then press SHiFT+R[or open Edit>Repeat last/ or Edit>Repeat History and choose last action] - and repeat action - all looks good. Then take any other side of the cube and press SHIFT+R and you will see that extrusion will oriented toward first one extrusion movement. Even if you will choose any other of transform orientation, you will never get correct result. It's works fine in old version of Blender.
