This report does not contain all the requested information, which are required for us to investigate the issue.
Please submit a new report through Help > Report a bug in Blender and carefully follow the submission template. Be sure to provide system information (operating system, GPU, driver version), Blender version, a short and precise description of the issue, step by step instructions to reproduce it and a .blend file that showcases the problem.
Update the graphics driver to the most recent version and check if the issue still occurs. If it does, open Blender's installation directory and double click on the blender_debug_gpu.cmd. This will start Blender in debug mode and create log files. Try to make Blender crash again. The Windows Explorer should open once Blender is terminated and show you two files. Add them to your bug report by clicking on the upload button as shown in the screenshot below.
Please note that we can only investigate and fix this issue, when it's a bug in Blender. We do not provide support for a third-party add-on like Radeon ProRender. Any issues with the add-on need to be reported to the respective developer/company.
